Behavioral health (or mental health) includes your emotional, psychological and social health. It affects how you think, feel and act, and impacts all areas of life. Some of the concerns that behavioral health professionals handle include: depression, anxiety or stress, anger management, alcohol or drug abuse, coping with grief or loss and more.

How to find a primary care provider (PCP) in our network

With the SignatureValue Harmony HMO plan, you'll need to select a primary care physician (PCP) for yourself and each covered member. Your PCP — who is in the Canopy Health network — can help guide you to the care that best fits your needs.

 

Choosing a network PCP

To get started, go to Provider Search. After that, follow these steps.

  1. Under “What type of Medical Care can we help you find near”, click Change Location, fill in your location information, and click Update Location.
  2. Under “Find Health Care by Category”, select People > Primary Care > All Primary Care Providers to see a listing of PCPs.
  3. Scroll through the listing and select the name of the network PCP you'd like to coordinate your care.
  4. Click on the PCP’s name for the 14-digit Provider ID number — write it down so that you can provide that information when you enroll.

Friendly reminders: If you're enrolling in a family health plan, please be sure to choose a PCP for each plan member. If you don't select a PCP by the deadline, one will be selected for you — but know that you can change your PCP anytime.
