SignatureValue Harmony HMO Plan

Designed to help give you a simpler health care experience, the UnitedHealthcare SignatureValue Harmony HMO plan enables you to choose from the SignatureValue network of providers — and work with a primary care physician (PCP) — who can help guide you to better health at lower costs by helping you get quality care. 

You and each covered family member will need to select a network PCP when you enroll. 

Preventive care is covered at 100 percent for age-appropriate care services.  

To enroll in this plan, you must live in Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Santa Cruz, SolanoHPD1 or Sonoma counties.

With this plan, you'll need to select a primary care provider (PCP) for yourself and each covered family member. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

If you don't select a PCP by the deadline, one will be selected for you.  But you can always change your PCP anytime at myuhc.com.
