UnitedHealthcare's SignatureValue Harmony plan is designed to give you a simpler health care experience that fits your needs. With this HMO plan, you choose from the Canopy Health network of providers — and work with a primary care physician (PCP) — who can help guide you to better health at lower costs by helping you get quality care. Your PCP will be your first contact — someone who can help guide you toward better health, coordinate your care and refer you to specialists, if needed.

This plan is designed to offer a streamlined experience with easier ways to find a doctor, more convenient ways to fill prescriptions, and online tools to help you get familiar with your benefits.

To enroll in SignatureValue Harmony in Northern California, you must live in Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Santa Cruz, Solano* or Sonoma counties.
