With this HMO plan, you choose from the Canopy Health network of providers — and work with a primary care physician (PCP) — who can help guide you to better health at lower costs by helping you get quality care.

You and each covered family member select a network PCP at enrollment and you can easily get a referral to see one of our high quality specialists. The Canopy Health Alliance Referral program offers seamless referrals throughout the Canopy Health network. 

Preventive care is covered at 100% for age-appropriate care services. 

To be eligible for this plan, you must live in Alameda, Contra Costa, Marin, San Francisco, San Mateo, Santa Clara, Santa Cruz, Solano* or Sonoma County.*

Network coverage

Network coverage plays a big part in delivering you cost savings and quality care. With the network, you'll have access to a group of health care providers and facilities that have a contract with UnitedHealthcare. Finding providers in the network is easy when you use Provider Search.

Primary care physician

With this plan, you'll need to select a primary care physician (PCP) for yourself and each covered member. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

It's easy to find and choose a network PCP — to get started, go to Provider Search. If you end up not picking a PCP by the deadline, one will be selected for you — but know that you can always change your PCP anytime.
