Eye health is important to you and your family’s overall health. A vision plan from UnitedHealthcare helps make it easy to maintain healthier eyes and good sight while helping you save money. Our vision plan provides eye exams and eyeglasses or contacts as well as discounts on popular lens options, laser vision correction, and extra pairs of eyewear. You’ll have access to a national network of providers that includes a mix of ophthalmologists and optometrists in both private practice and retail settings.

Important information	For frames that exceed the allowance, an additional discount may be applied to the overage. Discounts and price protections may not be available at all providers. Please confirm cost and coverage with your provider before making a purchase.

Elective contact lenses benefits

Important information	Contact lenses may be selected instead of eyeglasses. The allowances shown here are applied toward the purchase of contact lenses. The contact lens copay is waived.

Necessary contact lenses benefits

Selection contact lenses	The fitting/evaluation fees, contact lenses, and up to two follow-up visits are covered in full after copay. If you choose disposable contacts, up to 6 boxes are included when obtained from a in network provider.
Non-selection contact lenses	A $150 allowance is applied towards the purchase of contact lenses outside the selection list. The allowance is for materials. No portion will be applied to the fitting and evaluation. Contact lens copay is waived.
Necessary contact lenses	Paid in full after copay
Important information	Selection contact lenses refer to contact lenses available on our selection contact list. Non-Selection contact lenses are contact lenses that are not on this list. The selection contact list can be found at myuhcvision.com. Necessary contact lenses are determined at the provider’s discretion for certain conditions. If your provider considers your contacts necessary, you should ask your provider to contact UnitedHealthcare Vision to confirm the reimbursement that UnitedHealthcare will make before you purchase such contacts.

Maternity and Children’s Vision Benefits

Eye or vision changes may be more frequent for those who are pregnant or breastfeeding — and for growing kids, too. With the Maternity and Children’s Vision Benefits, expanded benefits are available to covered individuals who are pregnant or breastfeeding, as well as covered children up to age 19.

Benefits include a second eye exam during the plan year if you notice changes to your eyes. There is also coverage for 1 new pair of glasses (frames and lenses) if your prescription changes 0.4 diopter or more in a plan year. There is no additional premium; standard copays apply.

When you use a network provider, you have an allowance to help purchase any frames your eye doctor offers. You’ll get a discount on any cost over the allowance amount.

Popular lens options like UV protection or anti-reflective coating are available to you at price-protected amounts. Plus, standard scratch coating and polycarbonate lenses for dependent children are available at no cost. Enjoy discounts on additional pairs of eyeglasses, including prescription sunglasses.
