Colorado Doctors Plan® is a different kind of health plan that focuses on your relationship with your doctor.
With this health plan, you will need to select a primary care provider (PCP) to guide your care, help you avoid cost surprises and support you in achieving your best health. Colorado Doctors Plan uses your personal health record to help network doctors coordinate your care.
If you need to see a network specialist, you will not need a referral from your PCP.
This plan includes copays - which is the fixed amount you pay each time you see a network provider.
Colorado Doctors Plan is available to employees living in one of these counties: Adams, Arapahoe, Boulder, Broomfield, Denver, Douglas, Elbert, El Paso, Jefferson, Larimer, Logan, Morgan, Phillips, Sedgewick and Weld counties.

You can find nearby network care by visiting the Doctors Plan provider directory on the UnitedHealthcare® app or myuhc.com® and search by location. You have access to the Doctors Plan PCPs, specialists and hospitals in any state — for any eligible service — regardless of your PCP local network selection.
Network coverage plays a big part in delivering you cost savings and quality care. With the network, you'll have access to a group of health care providers and facilities that have a contract with UnitedHealthcare.

With this plan, you'll need to select a primary care provider (PCP) for yourself and each covered family member. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.
If you don't select a PCP by the deadline, one will be selected for you. But you can always change your PCP anytime at myuhc.com.
