Medical plans overview

UnitedHealthcare offers a variety of health plans with different provider networks. This means that doctors and hospitals may vary based on the health plan you choose. 

Please be sure to review your benefits documents for more information about your plan and to find out the provider networks associated with your plan.

Doctors and facilities in the UnitedHealthcare network have agreed to provide services for you at a discount. Try to use network providers whenever you can because it can help lower your costs.

That way, you can find care that is right for you and your family, and may even save money.

Dental plans overview

UnitedHealthcare offers a variety of dental plans designed to help you pay for dental check-ups, cleanings and other services to help keep your teeth and gums healthy. 

An example of a dental plan available through UnitedHealthcare is highlighted below: our Dental Participating Provider Organization (PPO) plan. 

Please review your enrollment materials and official plan documents to see if your employer offers a dental plan and the type of plan offered. 

Dental PPO highlights

  • See any dentist you want, anywhere across the country. When you choose a network dentist, you may receive discounted rates only available to members. Out-of-network providers may charge more for services, even above what your plan allows.  Please note that if a network dental provider is not available within a reasonable distance of where you live or work, you may be allowed to see an out-of-network dental provider and still receive services at the network rate.
  • Get coverage on hundreds of services, such as crowns, braces, silver fillings and more.
  • Referrals are not needed to see a specialist.
  • Preventive care is covered 100% in the network. Includes routine cleanings, fluoride treatments and bitewing X-rays.
  • Expectant mothers get extra coverage. Cleanings can be received during pregnancy and three months following delivery (This benefit not available in all states).
  • Oral cancer screenings are covered for adults (age 18 and older).
  • Earn award dollars by getting dental checkups.

Paying for care with a Dental PPO

  • You pay a deductible. This does not apply to preventive care services received in the network.
  • You pay coinsurance. This does not apply to preventive care services received in the network.
  • Your plan pays until it reaches the annual maximum, the most your dental plan will pay for covered services in a plan year.

Provider network options with Dental PPO

The Dental PPO plan can be offered with one of two national networks of dentists and dental clinics. If your employer offers Dental PPO, please see your plan documents for the provider network being used.

If you already have a physician, you may also wish to confer with him or her for advice on selecting other physicians. You should also discuss designations with a physician before choosing him or her. Dental PPO: This policy DOES NOT include coverage of pediatric dental services as required under federal law. Coverage of pediatric dental services is available for purchase in the State of Colorado, and can be purchased as a stand-alone plan, or as a covered benefit in another health plan. Please contact your insurance carrier, agent, or Connect for Health Colorado to purchase either a plan that includes pediatric dental coverage, or an Exchange-qualified stand-alone dental plan that includes pediatric dental coverage.
