The doctors and facilities in the SignatureValue® Harmony Plan network have agreed to provide services at a discount, which may help lower your health care costs.
The UnitedHealthcare SignatureValue® Harmony HMO health plan is designed to deliver lower costs, more choice and a simpler experience. This network allows members to choose from 16,000+ network physicians and 150+ hospitals1.
Go anywhere in the SignatureValue Harmony HMO network
The network includes the following provider groups. Each group offers primary/specialty care, urgent care, imaging, surgery, and other services. Use the links below to learn more about the unique features each offers.

Behavioral health (or mental health) includes your emotional, psychological and social health. It affects how you think, feel and act, and impacts all areas of life. Some of the concerns that behavioral health professionals handle include: depression, anxiety or stress, anger management, alcohol or drug abuse, coping with grief or loss and more.
Select a behavior health provider or facility that's in our network.
With the SignatureValue Harmony HMO plan, you'll need to select a primary care physician (PCP) for yourself and each covered member. Your PCP — who is in this network — can help guide you to the care that best fits your needs.
It's easy to find and choose a network PCP
To get started, go to Provider Search. After that, follow these steps.
1. Under Find Health Care by Category, select People.
2. Then select Primary Care for a list of the PCP types.
3. Click on All Primary Care Physicians to see a list of PCPs.
4. Choose the name of the network PCP you'd like to coordinate your care.
5. Click Enrollment Information for the Physician ID number— you'll need this number to enroll.
A couple friendly reminders: If you're enrolling in a family health plan, please be sure to choose a PCP for each plan member. If you don't select a PCP by the deadline, one will be selected for you — but know that you can change your PCP anytime.
