Many doctors and hospitals

We offer one of the largest national physician and hospital networks available. Our network includes more than 1.7 million doctors and health care professionals and 5,500+ hospitals1.

Selecting a primary care physician (PCP) is not required

See your plan documents to find out if you need a referral to see a network specialist.

Get guidance searching for providers

When searching the network directory, look for the blue hearts beside the provider’s name. The blue hearts identify UnitedHealth Premium providers across various specialties who meet quality care criteria, which includes safe, timely, effective and efficient care.

Search for network providers

The doctors and facilities in our plan networks have agreed to provide services at a discount, which may help lower your health care costs.

Your member identification (ID) card will show the plan network being offered. Using the list below, select that plan’s network link to find a medical provider or health care facility in the network.

W500 network is an additional network benefit available only with the UnitedHealthcare Core Network. Verify the network being offered as shown on your member ID card.

Please check your plan documents to confirm you have direct access to UnitedHealthcare Behavioral Health providers within your plan.

Excluded care providers

Routine Vision services are excluded from the plan network. This exclusion includes, but is not limited to:

  • Routine vision exams
  • Refractions
  • Hardware

Routine Hearing services are excluded from the plan network. This exclusion includes, but is not limited to:

  • Routine hearing exam
  • Routine hearing test
  • Hearing aid, batteries and accessories

Important: Be sure to check your specific plan benefits for vision and hearing coverage. When searching the network directory, you may see vision and hearing providers listed who are contracted for medical services but not routine vision and hearing services.

Our hearing program offers high quality, custom-programmed hearing aids at significant discounts. It also includes:

  • No-cost hearing exam and evaluation
  • Access to more than 6,500 providers nationwide
  • High quality hearing aids for a fraction of the cost (30-50% off MSRP)

  • Comprehensive hearing exam and evaluation
  • 3 follow-up visits
  • 1 time loss & damage replacement
  • 3-yr warranty
  • Extra batteries or charging case

Important: To get discounts and services through this program, you’ll need to obtain a referral letter from UnitedHealthcare Hearing before your appointment. This tells the provider you’re covered and to go through UHC Hearing instead of your Health Benefit Plan; it may also help lower your out-of-pocket costs.

Other routine services may be covered under your plan, and some plans may require copayments, coinsurance or deductibles for these benefits.
