Plans and network providers

Accountable Care Plan

The Accountable Care Plan is a network-only plan available to employees who live in certain ZIP codes in Arizona, ArkansasFlorida, Iowa, Louisiana, Mississippi, Nebraska, Ohio, Oregon, Rhode Island, Tennessee, Texas, Virginia, Washington and Wisconsin. You will get the highest level of benefits when you use a Tier 1 provider.

Charter Copay Plan

The Charter Copay Plan is a network-only plan available to employees who live in certain ZIP codes in the Houston, Texas, Indianapolis, Indiana and Chicago, Illinois areas.

M Health Fairview Primary Care Plan

The M Health Fairview Primary Care Plan is a network-only plan available to eligible employees who live in select ZIP codes in Minneapolis and St. Paul, Minnesota.

WellMed 1st Tier Plan

The WellMed 1st Tier Plan is a network-only medical plan available to eligible employees who live in certain ZIP codes in the San Antonio, Texas, service area and work in a specific WellMed business segment.

HSA Plans

The Low-deductible and Low-premium plans are available to eligible employees in all U.S. locations except Hawaii.

An HSA is designed to work with an IRS-qualified Consumer-Directed Health Plan (CDHP), including the UnitedHealth Group HSA plans. Contributions go in tax-free, the money grows income tax-free and comes out income tax-free when you use it for eligible medical expenses. The company also contributes to your HSA based on your coverage level.

Hawaii PPO plan

The Hawaii PPO plan is available to employees who live in Hawaii.

Behavioral health providers

Behavioral health (or mental health) includes your emotional, psychological and social health. It affects how you think, feel and act, and impacts all areas of life. Some of the concerns that behavioral health professionals handle include: depression, anxiety or stress, anger management, alcohol or drug abuse, coping with grief or loss and more.

Dental plan

This plan gives you freedom to see any dentist nationwide and saves you money when you see a dentist in the PPO network.

Vision plan

A vision plan from UnitedHealthcare provides access to a national network of providers. This network includes optometrists and ophthalmologists as well as the choice of either a private practice or retail setting.

This is a benefit of diabetes medications and supplies that are covered at a $0 cost share when filled at Optum home delivery. Some medications may have requirements such as prior authorization.

With a 3-month supply, you may be less likely to miss a dose. You can even sign up for automatic refills. Once you enroll and want to start Optum Home Delivery, call the number on your member ID card or visit myuhc.com® or the UnitedHealthcare® app to set up automatic refills.

Earn up to $100 in Rewards for Health

Earn up to $100 in Rewards for Health if you opt to use Optum Home Delivery Pharmacy for the first time between Jan. 1, 2025 and Dec. 31, 2025 (restrictions apply).

Pay your way 1 month at a time

Make 1 payment upfront or split it up into 3 equal monthly payments with the Easy Payment Plan.  Once you enroll and want to start Optum Home Delivery, call the number on your member ID card to arrange this payment.

Prescription drugs — how to find out what's covered

When it comes to coverage, keep in mind that some medications may have requirements before filling them. Also, if a medication doesn't fit your budget, it's a good idea to remember your doctor is here to help—talk to them about lower cost options.

No out-of-pocket costs on preferred insulin and other vital drugs

UnitedHealthcare is taking an important step in its continued efforts to make prescription drugs more affordable by eliminating out-of-pocket costs for drugs that are critical to maintaining the health of our members.

UnitedHealthcare’s Vital Medication Program offers the preferred drugs listed below at no out-of-pocket cost to you.

  • insulin – rapid, short and long-acting
  • epinephrine – allergic reactions
  • glucagon – hypoglycemia (low blood sugar)
  • naloxone – opioid overuse
  • albuterol – asthma

Prescription Drug List

The PDL includes brand and generic prescription medications approved by the FDA. Medications are placed in four different "tiers" based on their overall value. This offers choice and provides lower-cost options. Tier 1 and Tier 2 medications have the most affordable cost-share amounts. Tier 3 or Tier 4 medications have higher cost shares. Most Tier 4 medications have a Tier 1 or Tier 2 lower-cost option. 

The pharmacy benefit plan highly encourages generics when a brand has a generic, consider the generic as your lowest cost option for prescription needs.*


    You will pay more if a brand with a generic is selected for any reason (an additional charge is applied with purchase of a brand with a generic.)

    Preventive Drug List

    To see if your medication is considered a preventive drug, please check the preventive drug lists:

    Health care regulations (Affordable Care Act) allow for certain preventive medications and supplements to be available to you at no cost. Please review the drug lists for medications that are covered at 100% with $0 copay when prescribed by a health care professional, age and/or condition appropriate and filled at a network pharmacy:

    Specialty medications

    Specialty medications are typically high cost and are used to treat complex conditions. The Specialty Pharmacy Program gives you access to pharmacists around the clock, cost-savings information, and much more. 

      Price your medications

      The Pricing Tool is being updated. Please check back in December to get the exact pricing for your medication(s). In the meantime, you can use the Pharmacy Drug List to find your medication tier. Depending upon the plan you select, the below prescription drug copay changes for 2026 may apply:

      Retail

      • Tier 1: $15
      • Tier 2 (change): $60
      • Tier 3 (change): $150
      • Tier 4: $300

      Optum Home Delivery

      • Tier 1: $35
      • Tier 2 (change): $150
      • Tier 3 (change): $375
      • Tier 4: $750

      UHG
      The copays above only apply for preventive drugs included in the Core Plus Preventive Drug List under the Accountable Care, Charter Copay, HSA Plans (Low-Deductible and Low-Premium), and M Health Fairview Primary Care plans; non-preventive drugs are subject to the medical plan deductible and coinsurance under these plans.

      The copays above apply for preventive and non-preventive drugs under the Choice Transition, Hawaii PPO, Surest Copay, and WellMed 1st Tier plans.

      We use the Optum Rx Select Pharmacy Network which includes 50,000 pharmacies in major drug stores, mass retailers, and supermarkets, as well as local pharmacies, home delivery and specialty pharmacy services. This network offers choice and convenience. Here are some key features:

      • Includes Walgreens and other large retail chains including most Walmart, Rite Aid, Costco locations, store pharmacies (such as Kroger, Publix and Albertsons/Safeway) and local pharmacies.
      • Optum home delivery.
      • Applies to all UnitedHealth Group plans except UnitedHealthcare of Nevada POS, UnitedHealthcare of Nevada HMO, SignatureValue HMO and SignatureValue Harmony HMO.
