The Accountable Care Plan is a network-only plan available to employees who live in certain ZIP codes in Arizona, Arkansas, Florida, Iowa, Louisiana, Mississippi, Nebraska, Ohio, Oregon, Rhode Island, Tennessee, Texas, Virginia, Washington and Wisconsin. You will get the highest level of benefits when you use a Tier 1 provider.
The Charter Copay Plan is a network-only plan available to employees who live in certain ZIP codes in the Houston, Texas, Indianapolis, Indiana and Chicago, Illinois areas.
The M Health Fairview Primary Care Plan is a network-only plan available to eligible employees who live in select ZIP codes in Minneapolis and St. Paul, Minnesota.
The WellMed 1st Tier Plan is a network-only medical plan available to eligible employees who live in certain ZIP codes in the San Antonio, Texas, service area and work in a specific WellMed business segment.
The Low-deductible and Low-premium plans are available to eligible employees in all U.S. locations except Hawaii.
An HSA is designed to work with an IRS-qualified Consumer-Directed Health Plan (CDHP), including the UnitedHealth Group HSA plans. Contributions go in tax-free, the money grows income tax-free and comes out income tax-free when you use it for eligible medical expenses. The company also contributes to your HSA based on your coverage level.
The Hawaii PPO plan is available to employees who live in Hawaii.
These plans are network-only plans (with the exception of the Nevada POS plan which combines elements of an HMO and PPO type plan) and are available to employees and their families who live in select ZIP codes.

The PDL includes brand and generic prescription medications approved by the FDA. Medications are placed in four different "tiers" based on their overall value. This offers choice and provides lower-cost options. Tier 1 and Tier 2 medications have the most affordable cost-share amounts. Tier 3 or Tier 4 medications have higher cost shares. Most Tier 4 medications have a Tier 1 or Tier 2 lower-cost option.
The pharmacy benefit plan highly encourages generics when a brand has a generic, consider the generic as your lowest cost option for prescription needs.*
