UnitedHealthcare SignatureValue® Harmony HMO Plan

UnitedHealthcare SignatureValue Harmony HMO is designed to give you a simpler health care experience that fits your needs. With this plan, you'll have access to Harmony's network of doctors — and work with a primary care physician (PCP) — who can help guide you to better health at lower costs by helping you get quality care.  

Using the Harmony network, you can choose from providers throughout California. In Northern California, go anywhere for care within the Canopy Health Network. In Southern California, providers include Sharp HealthCare, UC San Diego Health, Optum (previously AppleCare Medical Group, HealthCare Partners and Monarch Healthcare), PrimeCare Chino, Beaver Medical Group, and the MemorialCare Medical Group. 

You won’t have coverage if you go out of the network or if you see a specialist without an electronic referral from your PCP. 

Preventive care is covered 100% in the network. You don't have to pay any out-of-pocket costs for age-appropriate preventive care as long as you use a network doctor. 

You are eligible to enroll in this plan if you reside in Los Angeles, Orange, Riverside, San Bernardino or San Diego County — or Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, and Santa Cruz Counties as well as parts of Solano or Sonoma County.

The importance of a primary care provider (PCP)

Primary care provider

With this plan, you'll need to select a primary care provider (PCP) for yourself and each covered family member. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

If you don't select a PCP by the deadline, one will be selected for you.  But you can always change your PCP anytime at myuhc.com.
