With SignatureValue Harmony plans

You'll have access to Harmony's network of doctors — and work with a primary care physician (PCP) — who can help guide you to better health at lower costs by helping you get quality care. Your PCP will be your first contact — someone who can help guide you toward better health, coordinate your care and refer you to specialists, if needed.

You are eligible to enroll in this plan if you reside in Los Angeles, Orange, Riverside, San Bernardino or San Diego County — or Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Santa Cruz, Solano* or Sonoma County.

SignatureValue® Harmony HMO - Southern California

The UnitedHealthcare SignatureValue® Harmony HMO plan is designed to help you navigate the health care system easier — while providing access to doctors who can help guide you to better health and lower costs. 

With this plan, you have access to an extensive network of contracted participating primary care physicians (PCPs) and other health care providers. Your PCP will be your first contact — someone who can help guide you toward better health, coordinate your care and refer you to specialists, if needed.

You are eligible to enroll in this plan if you reside in Orange, Los Angeles, Riverside, San Bernardino or San Diego County.

SignatureValue® Harmony HMO - Northern California

In collaboration with Canopy Health, UnitedHealthcare SignatureValue® Harmony plan is designed to give you a simpler health care experience that fits your needs.

With this HMO plan, you choose from the Canopy Health network of doctors — and work with a primary care physician (PCP) — who can help guide you to better health at lower costs by helping you get quality care. 

You are eligible to enroll in this plan if you live in Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Santa Cruz, Solano* or Sonoma County.
