Dental HMO Plan - You can visit any in-network dentist with no deductible or annual max. Preventive care is covered 100%. No referral is needed for specialists, but without one, you’ll get 25% off the standard fee. Out-of-network coverage is limited to preventive and diagnostic services.
Dental PPO Plan - You can see any dentist nationwide. Choosing an in-network provider gives you access to member-only discounts. No referral is needed for specialists, and preventive care like exams and cleanings is covered 100% in-network.
