You can see any dentist in our network. There’s no deductible and no annual maximum, which is the limit to how much the plan will pay each year. Most preventive dental care, including routine checkups and cleanings, is covered 100% in the network.

Please keep in mind: 

• A referral is not required to see a specialist; however, without one, you will receive a 25% reduction in the specialist's usual and customary fee. 
• You must use Network Providers. While you have some out-of-network coverage, that coverage is for Preventive and Diagnostic Services only.
