Selecting a primary care physician (PCP) is not required

The doctors and facilities in our network have agreed to provide services at a discount, which may help lower your health care costs. 

Please note: If your provider does not show in the results they are likely out of network. Please call 800-860-5203 to speak to a UnitedHealthcare representative and verify their participation in the Choice Plus Network.
