Choice Plus

With the Choice Plus Plan, you can use any doctor, clinic, hospital or health care facility in our national network, which helps save you money. From within this national network you can select Tier 1 doctors and specialists to help you get the most from your benefits. Be sure to check the network as our Tier 1 provider list is updated annually.

You can also use freestanding facilities for certain services to help with additional cost savings.

There’s coverage if you need to go out of the network. However, seeing an out- of-network provider will likely cost you more.

A referral is not required to see a specialist.

Age-appropriate preventive care is covered 100% as long as you use a network doctor.

Tier 1 providers

Spot the dot to find Tier 1 providers. These providers may help you pay less for care because they offer the greatest value from your health care benefits. They also meet national standards for safe, timely and effective quality care.

Freestanding facilities

Using a freestanding facility instead of a hospital may help you save money. These medical facilities include radiology centers and surgery centers/ambulatory surgery centers.

The importance of a primary care provider (PCP)

Primary care provider

This plan does not require you to have a primary care provider (PCP), but choosing one for yourself and each covered family member is highly recommended. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

Easily find and choose a primary care provider
Click the provider network link above to open the network provider directory for your selected plan.

Then follow these steps:

Under “What type of Medical Care can we help you find near:”, click Change Location, enter your location information, and click Update Location.
Under “Find Care by Category”, select People > Primary Care > All Primary Care Providers to see a listing of PCPs.
Scroll through the listing and select the name of the network PCP you'd like to coordinate your care.
Click on the PCP’s name for the Provider ID number.

Important: For health plans requiring a PCP, a PCP will be selected for you and your covered family members if you don’t pick one by the enrollment deadline. You can change your PCP anytime at myuhc.com.
