UnitedHealthcare (UHC) Network Plan

With this health plan, you can use any doctor, clinic, hospital or health care facility in the choice network, which saves you money. 

If you don’t use a provider from our network, you’ll have to pay for all of the costs. Referrals are not required to see a specialist.  

Preventive care is covered 100 percent in the network. You don't have to pay any out-of-pocket costs for age appropriate preventive care as long as you use a network doctor.

Health plan note(s):

  1. Massage Therapy is covered at $30 copay per visit up to 30 visits per calendar year subject to medical necessity.  
  2. Virtual Visits are covered at 100%. Your cost is $0 when you use one of the provider groups in the UnitedHealthcare 24/7 Virtual Visits network. If you receive services in addition to office visit, additional copays or coinsurance may apply. No virtual visit coverage for out-of-network.
    Copays will apply to virtual primary care.
