Primary care provider

For Colorado Doctors Plan, you'll need to select a primary care provider (PCP) for yourself and each covered member. While the Choice Plus HDHP plan does not require you to have a primary care provider (PCP), choosing one for yourself and each covered member is highly recommended.

Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

How to choose your PCP
Go to the Search for a Provider tab and click the button for your selected plan.

Under “What type of Medical Care can we help you find near”, click Change Location, fill in your location, and click Update Location.
Under “Find Health Care by Category”, select People > Primary Care > All Primary Care Providers to see a list of network PCPs.
From the list, select the network PCP you'd like.
Click on the PCP’s name for the 14-digit Provider ID number — write it down.
Email your selection including the provider ID number to DPSenrollment@uhc.com.

Important: A PCP will be selected for you if you don’t select one by the enrollment deadline. You can also change your PCP anytime at myuhc.com.
