Concierge Care Team

Health care can be complicated, so we're reinventing the way our plan participants think about health care customer service. The Concierge Care Team is here to help make it easier on you and your family. Here are a few ways we can help: 

  • Need a high-quality network provider? We’ll find one and even make your appointment or send you an email with a list of providers in your local area.
  • Questions about an unexpected bill or claim?  We’ll call your provider and figure it out — and then call you back with answers.
  • Dealing with a complex medical issue like autism? We’ll connect you to resources to help support you with the emotional and financial stress of a diagnosis and treatment.
  • Can’t find your health plan ID card? We can send you a new one and show you how to access a copy right on your phone using the UnitedHealthcare® app*. The app is available at no additional cost to you from your app store.
  • Frontline for Pharmacy navigation. Concierge team has access to OptumRx information and provides a single point of contact for members. The team handles about 80% of pharmacy calls which provides a simpler experience and a greater opportunity for clinical referrals.

Transferring care

If you are new to UnitedHealthcare, there’s some important information and steps to consider before your plan goes into effect.

To help make your care transfer experience easier, start with this checklist of considerations:

  • Fill all open prescriptions with your current provider prior to your plan’s start date
  • Initiate a transfer of your prescriptions to a new network pharmacy if necessary
  • If switching providers, request a transfer of medical records prior to your start date
  • Consider whether you’ll need to continue active treatment for a chronic or acute condition
