Transition of care

If you are new to UnitedHealthcare coverage and are currently receiving treatment for a specific medical condition by an out-of-network provider, we don’t want to disrupt that care. That’s why we offer Transition of Care, which lets you continue to receive care - for a limited time - as we help you make the change to a network provider. 

To see if you qualify and to obtain a Transition of Care form, you can contact your benefit manager or call to speak with a UnitedHealthcare advocate. Be sure to advise them of your state of residence so that you are given the proper form.

With 24/7 Virtual Visits, you can connect to a care provider by phone or video* through myuhc.com® or the UnitedHealthcare® app. Providers can treat a range of non-emergency health conditions — and may even prescribe medication as needed**.

Fertility Solutions

The Fertility Solutions program connects you with specially trained nurses and other experts who will help educate you and give guidance on infertility treatment. They can also direct you to infertility experts and treatment through our Centers of Excellence clinics.

Pre-certification required: You must pre-certify all inpatient and most outpatient mental health services. Benefits for the following services will not be paid if not pre-certified: Autism treatment, Psychological testing, Biofeedback, Electroshock therapy, and Hypnosis. We strongly encourage you to call UBH at 888-249-6760 before receiving any mental health or substance abuse services to ensure coverage and avoid unnecessary out-of-pocket expenses.

Condition Management

If you're dealing with complex health care needs or living with a chronic condition such as asthma, coronary artery disease, congestive heart failure or diabetes, it can be difficult to sort through all the available information to make informed decisions. UnitedHealthcare can provide you with access to specialized nurses who will assess your needs and develop a personalized care plan to support you with symptom management, provide education on risks, and help you foster a healthy lifestyle.

Custom Care Management

If you or a covered spouse/domestic partner have a chronic condition such as asthma, diabetes, congestive heart failure, coronary artery disease, and low back pain there's a lot you can do to take charge of your health. By partnering with a specialty trained registered nurse you can learn about self-care, how to manage your medications and tips for working with your doctor more effectively. Plus, you can be rewarded with gift card incentives for being engaged in your own health management.
