Get support throughout your pregnancy

Whether it's your first child or your third, you need quality care during pregnancy. Get personalized guidance throughout your pregnancy through one-on-one access to an experienced maternity nurse, medical director, and social worker throughout your pregnancy, regardless of risk, to provide assistance, guidance, answers, and education.

The information provided through Advocate services is for informational purposes only and provided as part of your health plan. Wellness nurses, coaches and other representatives cannot diagnose problems or recommend treatment and are not a substitute for your doctor’s care. Your health information is kept confidential in accordance with the law. Advocate services are not an insurance program and may be discontinued at any time.

Certain preventive care services are provided as specified by the Patient Protection and Affordable Care Act (ACA), with no cost-sharing to you. These services are based on your age, gender and other health factors. UnitedHealthcare also covers other routine services that may require a co-pay, co-insurance or deductible.

Virtual Visits: The Designated Virtual Visit Provider’s reduced rate for a virtual visit is subject to change at any time.

Virtual Visits phone and video chat with a doctor are not an insurance product, health care provider or a health plan. Unless otherwise required, benefits are available only when services are delivered through a Designated Virtual Network Provider. Virtual Visits are not intended to address emergency or life-threatening medical conditions and should not be used in those circumstances. Services may not be available at all times or in all locations.

Participants should consult an appropriate health care professional to determine what may be right for them. Employers are responsible for ensuring that any wellness programs they offer to their employees comply with applicable state and/or federal law, including, but not limited to, GINA, ADA and HIPAA wellness regulations, which in many circumstances contain maximum incentive threshold limits for all wellness programs combined that are generally limited to 30 percent of the cost of self-only coverage of the lowest-cost plan, as well as obligations for employers to provide certain notices to their employees. Co-pay/Co-insurance waiver

If you are participating in and meeting the requirements of a disease management program for diabetes, asthma, or CAD, you may not have to pay any co-pay or co-insurance payments for certain medications used to treat that condition. Click on your plan below to learn more.

Family Support Program

Family Support Program (FSP) is a telephonic case management program to support the caregiver of a high needs’ pediatric member.  Caregivers of children between the ages of 0 – 18 with complex behavioral health conditions will receive a dedicated 1:1 FSP Care Advocate who will first assess the needs of the family, and then provide tailored navigation, education, guidance, referrals and support to address the specific family needs.

Get help managing Diabetes

The Diabetes Disease Management Program connects you with experts who can provide information to help you manage your diabetes, manage your medications, and improve your diet and exercise.

The program provides members with personalized, 1-on-1 support designed to address their needs throughout the care journey. Our case managers engage with SHBP members a minimum of every 30 days and every 90 days if their Diabetes is well controlled. Additionally, provider outreach is made to obtain clinical updates including notes, labs and other clinical indicators.

Our enhanced digital capabilities provide the right balance of education, tools and motivation as well as increased communication to help us more closely manage care for SHBP members. SHBP members can exchange messages with their nurse, access curated educational content and their nurse can also communicate on changes in health data (provided by employee) like blood pressure, weight and glucose.

Get help to breathe easier

The Asthma Disease Management program helps reduce unnecessary hospitalizations and health care costs, missed days of work or school and improve the member’s quality of life. The program provides information and resources to help participants:

  • Avoid triggers and recognize warning signs of an attack
  • Reduce or eliminate risk factors
  • Follow their medication regimen and treatment plan
  • Effectively manage their condition and co-morbidities, including depression

If you have Chronic Obstructive Pulmonary Disorder

The Chronic Obstructive Pulmonary Disease Program will help you manage your condition with:

  • A personal early detection plan to recognize respiratory symptoms
  • Programs that may help you to reduce risks like smoking and second-hand smoke exposure
  • Reports that are sent to your doctor if serious issues are detected
  • Support to help you manage high blood pressure, diabetes, Coronary Artery Disease and other conditions that can affect your COPD

Access a team of specialized Congenital Heart Disease nurses that will support you through all stages of treatment and recovery. They will help you make informed treatment decisions and guide you to top-performing Centers of Excellence providers.

Get help if you have kidney disease

If you’ve been diagnosed with kidney disease, the Comprehensive Kidney Solution program will provide resources to help you manage it. Our experienced nurses will help you learn how to manage the disease. They will also work with your doctor, dialysis centers, and others on your care.

Do you need a transplant?

If you need a transplant, the Transplant Resource Services program can connect you to transplant Centers of Excellance. Our experienced nurses will work with you and your doctors throughout your care.

Help is available for members with autism

Special support is also available if you or a covered dependent is diagnosed with autism. The Enhanced Autism Program is a unique program that includes Applied Behavior Analysis (ABA) therapy and provides a specialized autism care management team to personally help you and your family.

Bariatric Resource Services

If you are considering surgical treatment for obesity, you need to make some difficult and important decisions. Among the most important decisions are which treatment to get and where to get it. An experienced bariatric nurse will work with you to help you make informed decisions and find a Bariatric Center of Excellence for weight-loss surgery with better outcomes and fewer complications.

The Centers of Excellence (COE) program providers and medical centers are independent contractors who render care and treatment to health plan members. The COE program does not provide direct health care services or practice medicine, and the COE providers and medical centers are solely responsible for medical judgments and related treatments. The COE program is not liable for any act or omission, including negligence, committed by any independent contracted health care professional or medical center.

Disease Management programs and services may vary on a location-by-location basis and are subject to change with written notice. UnitedHealthcare does not guarantee availability of programs in all service areas and provider participation may vary. Certain items may be excluded from coverage and other requirements or restrictions may apply. If you select a new provider or are assigned to a provider who does not participate in the Disease Management program, your participation in the program will be terminated. Self‑Funded or Self‑Insured Plans (ASO) covered persons may have an additional premium cost. Please check with your employer.

 

Earn well-being credits

Well-being incentive credits and the UnitedHealthcare match

Well-being incentive credits you and your covered spouse redeem can be used for eligible medical and pharmacy expenses. You and your covered spouse can earn up to 720 credits each which includes the 240 UnitedHealthcare incentive credit match.

Using the Health Incentive Account (HIA) with the Choice HMO Plan

If you sign up for the Choice HMO, complete the wellness activities, and redeem your points through the Sharecare Redemption Center, your redeemed credits will be placed into your UnitedHealthcare HIA.

We will automatically use the money in your HIA to help pay for eligible medical and pharmacy expenses. This lowers the amount you have to pay.

Using the Health Incentive Account (HIA) with the HDHP with an HSA

If you sign up for the HDHP, complete the wellness activities, and redeem your points through the Sharecare Redemption Center, your redeemed credits will be placed into your UnitedHealthcare HIA. 

  • IMPORTANT: Before the money in the HIA can be used, you will need to pay for covered services until the following amounts have been paid toward your deductible. 
    • You - $1,400
    • You + Child(ren) - $2,800 
    • You + Spouse - $2,800
    • You + Family - $2,800
  • After you pay the above amount, we will automatically use the money in your HIA to help pay for eligible medical and pharmacy expenses. This lowers the amount you have to pay. 
  • The HIA will automatically be used until it is gone. Then you will need to pay any remaining amount out of your pocket. 
  • You can continue to save money in an HSA for services that the HIA may not pay for. Remember, your HSA can be used to save for expenses you’ll have in the future.

Earn Well-Being Points with Be Well SHBP®

Your points will be earned as activities are completed. To begin earning your points, complete the following activities between January 1, 2022 and November 30, 2022.

  1. Assess Your Health: Complete the RealAge Test: A confidential, online questionnaire that will take about 10 minutes to complete.
  2. Know Your Numbers: You have three options to complete your Biometric Screening: through your physician or at an SHBP-sponsored screening event or at a Quest Diagnostic Patient Service Center (PSC).
  3. Take Action: Complete the Coaching Pathway, Online Pathway, RealAge Program or a combination of all three. (Points to be earned after the RealAge Test is completed).
  • Well-being Coaching Pathway: Actively engage in well-being coaching. Earn 40 points for one completed coaching call per calendar month. You can earn 40 points up to 6 times, for a maximum of 240 points. Maximum of one call in a calendar month qualifies you for the 40 points.
  • Online Pathway: Actively track and make progress. Earn 120 points up to 2 times, for a maximum of 240 points.
  • RealAge Program: Earn 120 points one time for completing the RealAge Program.

NOTE: Points cannot be awarded until completion of the RealAge Test. Biometrics, Well-being Coaching, Online Pathways and RealAge Program taken before completion of the RealAge Test can only be applied to points upon RealAge Test completion.

How it works

Complete both 1 and 2 and earn 240 points from Sharecare.

Complete 3 and earn up to 240 points from Sharecare.

Employees and their covered spouse can each earn an additional 240 well-being incentive credits from UnitedHealthcare.

When do I receive my credits?

Choose from two options for redeeming points.

  • 480 well-being credits applied toward eligible medical and pharmacy expenses in your UnitedHealthcare Health Incentive Account (HIA). Please note that points can be redeemed in increments of 120.
  • A $150 Prepaid Visa® Card (All 480 points are needed for redemption).

Regardless of how you redeem your points through Sharecare, UnitedHealthcare will match up to 240 additional credits* for you and your covered spouse to be used in your HIA to help offset eligible medical and pharmacy expenses. These additional credits awarded by UnitedHealthcare cannot be used toward the redemption of the $150 Prepaid Visa® Card.

* The UnitedHealthcare incentive credit match will be available after you redeem your well-being credits through the Sharecare Redemption Center.

What can I use my credits for?

If you sign up for the Choice HMO: You can use your well-being incentive credits to help pay your co-payments, your deductible, and other out-of-pocket expenses for claims submitted electronically through your healthcare provider. Credits will be available in your HIA 30 days after redemption.

Important: For members enrolled in the HDHP, well-being HIA credits cannot be used until the $1,400 (employee) or $2,800 (employee + spouse, employee + child(ren), or employee + family) thresholds have been met.

Additionally, you can continue to save money in an HSA for services that the HIA may not be reimburse. Remember, your HSA can be used to save for expenses you’ll have in the future.

Rollover your well-being incentive credits to a UnitedHealthcare plan.

Regardless of which UnitedHealthcare plan you choose, your unused well-being credits will follow you. Whatever you earn by participating in the Be Well SHBP well-being program administered by Sharecare you get to keep. Rollover credits from a prior year cannot be used to redeem Prepaid Visa® Card.
