If you are participating in and meeting the requirements of a disease management program for diabetes, asthma, or CAD, you may not have to pay any copay or coinsurance payments for certain medications used to treat that condition. Click on your plan below to learn more.
The Diabetes Disease Management Program connects you with experts who can provide information to help you manage your diabetes, manage your medications, and improve your diet and exercise.
The program provides members with personalized, 1-on-1 support designed to address their needs throughout the care journey. Our case managers engage with SHBP members a minimum of every 30 days and every 90 days if their Diabetes is well controlled. Additionally, provider outreach is made to obtain clinical updates including notes, labs and other clinical indicators.
Our enhanced digital capabilities provide the right balance of education, tools and motivation as well as increased communication to help us more closely manage care for SHBP members. SHBP members can exchange messages with their nurse, access curated educational content and their nurse can also communicate on changes in health data (provided by employee) like blood pressure, weight and glucose.

Certain preventive care items and services, including immunizations, are provided as specified by applicable law, including the Patient Protection and Affordable Care Act (ACA), with no cost-sharing to you. These services may be based on your age and other health factors. Other routine services may be covered under your plan, and some plans may require copayments, coinsurance or deductibles for these benefits. Always review your benefit plan documents to determine your specific coverage details.
24/7 Virtual Visits is a service available with a Designated Virtual Network Provider via video, or audio-only where permitted under state law. Unless otherwise required, benefits are available only when services are delivered through a Designated Virtual Network Provider. 24/7 Virtual Visits are not intended to address emergency or life-threatening medical conditions and should not be used in those circumstances. Services may not be available at all times, or in all locations, or for all members. Check your benefit plan to determine if these services are available.

The Centers of Excellence (COE) program providers and medical centers are independent contractors who render care and treatment to health plan members. The COE program does not provide direct health care services or practice medicine, and the COE providers and medical centers are solely responsible for medical judgments and related treatments. The COE program is not liable for any act or omission, including negligence, committed by any independent contracted health care professional or medical center.
Disease Management programs and services may vary on a location-by-location basis and are subject to change with written notice. UnitedHealthcare does not guarantee availability of programs in all service areas and provider participation may vary. Certain items may be excluded from coverage and other requirements or restrictions may apply. If you select a new provider or are assigned to a provider who does not participate in the Disease Management program, your participation in the program will be terminated. Self‑Funded or Self‑Insured Plans (ASO) covered persons may have an additional premium cost. Please check with your employer.
You and your covered spouse can each earn up to 480 credits in your health incentive account (HIA), plus each earn a $250 Reward Card through UnitedHealthcare. Here’s how:
Your points will be earned as activities are completed. To begin earning your points, complete the following activities between January 1, 2024 and December 2, 2024.
1) Assess Your Health: Complete the RealAge Test: A confidential, online questionnaire that will take about 10 minutes to complete.
- Upon completion, 120 points will be earned
2) Know Your Numbers: You have three options to complete your Biometric Screening: through your physician or at an SHBP-sponsored screening event or at a Quest Diagnostics Patient Service Center (PSC).

3) Take Action: Complete the Well-being Coaching, Monthly Engagement Activities, Preventive Screening Exams, or a Combination of all Three (Points to be earned after the RealAge Test is completed).
• Well-being Coaching: Actively engage in well-being coaching. Earn 40 points for one completed coaching call per calendar month. You can earn 40 points up to 6 times, for a maximum of 240 points.
• Monthly Engagement Activities: Actively track and make progress. Earn 40 points up to 6 times, for a maximum of 240 points by completing the required activities within the monthly engagement timeframe.
• Preventive Screening Exams: Earn 60 points for each completed screening exam, up to two times.
NOTE: Points cannot be awarded until completion of the RealAge Test. Biometrics, Well-being Coaching, Monthly Engagement Activities, and Preventive Screening Exams can only be applied to points upon RealAge Test completion.
Complete activities 1 and 2 to earn 240 points from Sharecare.
Complete activity 3 to earn up to 240 points from Sharecare.
When do I receive my well-being credits and the UnitedHealthcare $250 Reward Card?
Choose from two options for redeeming well-being points.
Regardless of how you redeem your 480 points through Sharecare, UnitedHealthcare will reward you and/or covered spouse by mailing you a $250 Reward Card up to $500 per household. The Reward Card will be physically mailed within 8 weeks of redemption.
Using the Health Incentive Account (HIA) with the Choice HMO Plan
If you sign up for the Choice HMO Plan and redeem your well-being incentive credits through the Sharecare Redemption center toward the UnitedHealthcare HIA, your credits will be automatically used to offset co-payments, deductible and out of pocket expenses for eligible medical and pharmacy claims submitted electronically through your healthcare provided. Credits will be available in your HIA 30 days after redemption.
When you or your covered spouse redeem 480 well-being credits, you will receive a $250 Reward Card from UnitedHealthcare up to $500 per household. The Reward Card can be used to help offset medical, pharmacy or other daily expenses.

If you sign up for the HDHP and redeem your well-being incentive credits through the Sharecare Redemption center toward the UnitedHealthcare HIA, your credits will be automatically used to offset co-insurance, deductible and out of pocket expenses for eligible medical and pharmacy claims submitted electronically through your healthcare provided. Credits will be available in your HIA 30 days after redemption.
When you or your covered spouse redeem 480 well-being credits, you will receive a $250 Reward Card from UnitedHealthcare up to $500 per household. The Reward Card can be used to help offset medical, pharmacy or other daily expenses.
IMPORTANT: Before the money in the HIA can be used, you will need to pay for covered services until the following amounts have been paid toward your deductible.
After you pay the above amount, UnitedHealthcare will automatically reimburse for eligible medical and pharmacy expenses from your HIA.
Additionally, you can continue to save money in an HSA for services that the HIA may not be reimburse. Remember, your HSA can be used to save for expenses you’ll have in the future.

Rollover your well-being incentive credits to a UnitedHealthcare plan.
Regardless of which plan option you choose, your unused well-being credits will follow you. Whatever you earn by participating in the Be Well SHBP* well-being program administered by Sharecare you get to keep. Rollover credits cannot be redeemed for a $150 Visa® Prepaid Card or the UnitedHealthcare $250 Reward Card.
*Be Well SHBP, the SHBP well-being program, is administered by Sharecare.
