Transition of care

If you are new to UnitedHealthcare coverage and are currently receiving treatment for a specific medical condition by an out-of-network provider, we don’t want to disrupt that care. That’s why we offer Transition of Care, which lets you continue to receive care - for a limited time - as we help you make the change to a network provider. 

To see if you qualify and to obtain a Transition of Care form, you can contact your benefit manager or call to speak with a UnitedHealthcare advocate. Be sure to advise them of your state of residence so that you are given the proper form.

A big part of living a healthier life is detecting health issues early. Preventive care — such as routine wellness exams and certain recommended screenings and immunizations — is covered by most of our plans at no additional cost when you see network providers. A preventive care visit may be a good time to help establish your relationship and create a connection for future medical services.

With 24/7 Virtual Visits, you can connect to a care provider by phone or video* through myuhc.com® or the UnitedHealthcare® app. Providers can treat a range of non-emergency health conditions — and may even prescribe medication as needed**.

Find guidance for fertility treatment

Fertility Solutions connects you with clinically guided support and information that can help educate and offer guidance as you explore options for expanding your family. You can self-schedule a call with a fertility nurse, search for fertility Centers of Excellence (COEs) and learn about fertility support via 24/7 online learning.  

Once your health coverage is effective, get started by calling the phone number on your health plan ID card.

When a health care question comes up, we can help you get connected to a health care professional. He or she can help you with:

  • Choosing appropriate medical care.
  • Obtaining guidance for current symptoms 24/7 (via a clinician).
  • Finding doctors or hospitals that meet your needs and preferences.
  • Locating an urgent care center and other health resources in your area.

If you have a special condition, our Centers of Excellence (COE) network provides help finding a doctor and medical center as well as help with understanding your illness. COEs are intended to complement local hospital and physician care and offer benefits that may include:

  • Fewer complications 
  • Planned and coordinated care provided by a team of experts 
  • Higher survival rates, shorter stays and decreased costs 
  • Expanded treatment options

Get help if you have kidney disease

Cancer Resource Services is here to help connect you and your family with dedicated cancer nurses who can offer personalized support during diagnosis through treatment. They will help you understand your diagnosis and share information that may help you make more informed decisions about your care as well as find emotional support for you and your family members.

Cancer resources

If you’ve been diagnosed with kidney disease, the Comprehensive Kidney Solution program will provide resources to help you manage it. Our experienced nurses will help you learn how to manage the disease. They will also work with your doctor, dialysis centers, and others on your care.

Congenital heart disease resource services

Access a team of specialized Congenital Heart Disease nurses that will support you through all stages of treatment and recovery. They will help you make informed treatment decisions and guide you to top-performing Centers of Excellence providers.

Support for parents whose infants are admitted to the NICU

Neonatal Resource Services (NRS) provides a dedicated team of experienced neonatologists, neonatal intensive care unit (NICU) nurse case managers and social workers who can provide support and assistance during your infant’s admission to the NICU. The case manager will also provide discharge planning assistance and ongoing support post-discharge based on your infant’s needs.  

Members can take part in the NRS program by calling the member phone number on their ID Card.
