Doctors Plan

A collaboration between Canopy Health and UnitedHealthcare, Doctors Plan delivers savings, a simple experience and access. It’s a health plan that sets its sights on building a strong connection between you and your primary care physician (PCP)—so you’ve always got a doctor in your corner guiding you to the care you need and helping you access cost-effective care.

You and each covered family member will need to choose a PCP. If you need to see a network specialist, you will not need a referral from your PCP.

This plan includes copays which is the fixed amount you pay each time you see a network provider. For urgent care visits and online doctor visits, copays start as low as $0. 

To enroll, you must live or work in Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Santa Cruz, Solano* or Sonoma County.

Primary care provider

With this plan, you'll need to select a primary care physician (PCP) for yourself and each covered member. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

It's easy to find and choose a network PCP—to get started, go to Provider Search. If you end up not picking a PCP by the deadline, one will be selected for you—but know that you can always change your PCP anytime at myuhc.com.
